Bridging End-of-Life Gaps

The Hospice Network of Maryland is initiating a new effort to offer programs for other caregivers in Maryland who deal with end-of-life issues.  The effort is intended to strengthen relationships between community hospices, nursing homes, assisted living facilities, adult day care and other providers.  By bringing together colleagues from various facilities to engage in fellowship and professional conversations, HNM hopes to bridge the gaps that often have us working in different directions to meet the same common goal, quality end of life care

Hospice - Veteran Partnership

The Hospice-Veteran Partnership is a collaboration between the Hospice Network of Maryland and VISN-5, the Maryland area network of VA Medical Centers.  This collaboration will work to better serve the Veterans throughout the Maryland area and increase veterans' access to hospice and palliative care.

    "The Department of Veterans Affairs is committed to providing quality health care, including end of life care, to the nation’s veterans.

     As part of this commitment, the Hospice-Veteran Partnership (HVP) was created to promote community-based partnerships dedicated to increasing veterans' access to hospice and palliative care; to strengthen relationships between community hospices and VA providers; and, to educate veterans and their caregivers about end-of-life care options.

     In keeping with VISN 5’s Hospice-Veteran Partnership, we provide a vast array of excellent hospice and palliative care services at each of our facilities, including referrals to our community hospice partners, to ensure that we meet each veteran’s end-of life preferences."
